Clinic Registration

I/We give permission for our daughter to participate in 2026 - 2027 HNL Volleyball activities. I/We realize that such activity involves the potential for injury, which is inherent in all sports. The undersigned, parent(s) or guardian(s) of the child named above, a minor hereby authorizes the coach, or such substitute as he or she may designate as the agent for the undersigned. This authorization is given prior to any diagnosis or treatment known to be required in order to enable such coach or agents to act effectively in an emergency situation where I cannot be contacted. Should said coach or agents exercise their authorized consent hereunder upon the advice of a licensed physician, surgeon or dentist, I knowingly and voluntarily exonerate and release coach or agents from any and all liability for this action. I/We further release the coaches, agents, and volunteers of all liabilities, and I/We accept all responsibility for all obligations, financial or otherwise, which may result from injuries sustained while participating. I/We have no knowledge of any reason the applicant cannot participate in vigorous physical activity. I/We hereby authorize coaches to use my child's name, video, or photograph at any time and in any manner in connection with its advertising, publicity & public relations programs. The coaches reserve the right to dismiss any player whose conduct is unsatisfactory. The volleyball club activities is for well-adjusted players who treat others with respect and value each player's right to a positive experience.

The undersigned hereby certifies that the player identified below is physically fit, is sufficiently prepared or trained to participate in the 2026 - 2027 HNL Volleyball ("HNLV") activities and has not been advised to refrain from or to discontinue participation by a qualified medical professional. The undersigned certifies the player has no health-related reasons or problems which precluded the player from participating in the Tryout; The undersigned acknowledges and understands that the activities involve facilities and/or equipment which may or may not be owned, maintained, controlled or operated by HNLV, and that the player could sustain injury (minor to major, temporary or permanent) from same which could possibly result from the use of such facilities and/or equipment, or from the various Tryout activities themselves; The undersigned assumes all risk of the player participating in the Tryout activities, including and without any limitation, any risk that may arise from negligence or carelessness on the part of HNLV, its coaches, volunteers, and any authorized representative of HNLV; The undersigned hereby waives, releases and discharges from any and all liability, including but not limited to, liability arising from the negligence or fault of HNLV, its coaches, volunteers, and any authorized representative of HNLV, for the player's death, disability, personal injury, property damage, property theft, or actions of any kind which may arise from player's participation in the Tryout activities; and The undersigned shall indemnify and hold harmless HNLV, its coaches, volunteers, and any authorized representative of HNLV against any and all claims made as a result of the player's participation in the Tryout activities, whether or not caused by negligence or HNLV, its coaches, volunteers, and any authorized representative of HNLV.
